Medicare/Medicaid and most insurance providers pay 100% for home care because it
is so cost effective. Any cost not covered by other primary or secondary payers
shall be itemized and explained by the Agency before care is started.
What types of services does the staff do while in my home?
Care is ordered by the physician and implemented by the RN case manager. The
services they perform are:
RN case manager: coordinates the care treatment and communicates patient's
progress to the doctor;
Skilled/psychiatric nursing (RNs): implements in-home psychosocial
rehabilitation skills and plan of treatment for patients with psychiatric
diagnoses;
Home Health Aide (HHA): provides or assists with personal care;
Physical Therapist (PT): assists with mobility and rehabilitation therapies;
Occupational Therapist (OT): teaches hand and fine motor skills;
Infusion Therapist: provides intravenous therapy;
Medical Social Worker: assists in counseling and utilizing community resources;

The number of care providers needed will depend on your health problems and
diagnosis. The same staff will see you every visit.

How often will they come to see me?
The schedule of visits will be arranged with the patient and the family. The RN
and other team members will schedule visits around your needs as closely as
possible.
